Waiver of Liability

1. General Voluntary Participation Agreement and Waiver of Liability
I, the undersigned, understand that yoga and other fitness classes taught at Cityoga Yoga School of Yoga and Health LLC, as with any physical activity, carries a risk of injury, even serious or disabling, that is always present and cannot be entirely eliminated. I represent and warrant that I have no medical condition that would prevent my participation in fitness classes Cityoga Yoga School of Yoga and Health LLC,. In consideration for my participation in class, I agree to assume full responsibility for any and all risks, injuries, or damages, known or unknown, which I might incur as a result of my participation, and I knowingly, voluntarily, and expressively waive any claim I might have against Cityoga Yoga School of Yoga and Health LLC, and any of its instructors.

2. Assumption of the Risk, Waiver of Liability, and Studio Policy Compliance

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at Cityoga Yoga School of Yoga and Health LLC, or any venue Cityoga provides any type of classes (“Claims”). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Cityoga Yoga School of Yoga and Health LLC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Cityoga Yoga School of Yoga and Health LLC, its employees, agents, and representatives.

By checking the box, I hereby agree to the General Voluntary Participation Agreement and Waiver of Liability, the Assumption of Risk, Waiver of Liability, and Studio Policy Compliance Agreement Relating to COVID-19, and the Broadcast Waiver, without exception or exclusion.
